Second Chance Body Armor — voluntary recall of Ultima, Ultimax, Monarch models

Second Chance (now defunct) issued a voluntary recall of all Zylon-containing soft armor. Approximately 98,000 vests worldwide were subject to replacement or refund. The company filed for bankruptcy in October 2004; recall fulfillment was taken over by Armor Holdings.
