Second Chance (now defunct) issued a voluntary recall of all Zylon-containing soft armor. Approximately 98,000 vests worldwide were subject to replacement or refund. The company filed for bankruptcy in October 2004; recall fulfillment was taken over by Armor Holdings.

Following a year-long investigation, NIJ withdrew compliance status from every body armor model containing any amount of Zylon (poly-p-phenylenebenzobisoxazole, PBO). Officer Zeppetella's Second Chance Ultima vest with Zylon (PBO) ballistic fiber failed to stop a 9mm round during his murder in 2003.
